Fixing Astigmatism with Laser Eye Surgery: Why "Iris Tracking" Matters

If you have astigmatism, your eye isn't perfectly round — it's shaped a bit like a rugby ball instead of a football. This means light entering your eye doesn't land on one clear point at the back of your eye. Instead it lands in more than one spot, which is what makes things look blurry or stretched.
Laser eye surgery fixes this by gently reshaping the surface of your eye. But here's the important part: it's not enough to reshape the right amount of eye. The laser also has to work in exactly the right direction. Think of it like sanding down a table leg to make it level — if you sand in the wrong direction, it won't come out straight, even if you sand the right amount off.
The Problem: Your Eye Moves a Little When You Lie Down
Here's something most people don't realise. When you go for your eye scans before surgery, you're sitting up. But when you actually have the surgery, you're lying flat on your back.
When your body position changes like this, your eye naturally twists slightly in its socket — just a small rotation, without you noticing or being able to control it. Doctors call this cyclotorsion.
This tiny twist matters a lot for astigmatism. Since the whole treatment depends on lasering in exactly the right direction, if your eye has quietly rotated a few degrees between your sitting-up scan and your lying-down surgery, the laser could end up working slightly off-target — unless the machine can detect and correct for it.
The Solution to Astigmatism: Iris Tracking
This is where iris tracking (also called iris registration) comes in.
Your iris — the coloured ring around your pupil — has a unique pattern, a bit like a fingerprint. Modern laser machines take a photo of this pattern during your pre-surgery scan while you're sitting up, and then take another photo right before treatment while you're lying down.
The machine compares the two photos, works out exactly how much your eye has rotated in between, and automatically adjusts the laser treatment to match — all before it starts firing. Some of the newest machines can even keep checking and adjusting throughout the whole treatment, not just at the start.
Which Machines Can Do This?
A few of the main laser systems used for this type of surgery include:
VISX STAR S4 — generally recognised as the first machine to do this automatically, using the iris photo-matching method described above, with no ink marks or manual guesswork needed.
WaveLight Allegretto Eye-Q — tracks eye movement at high speed and also has a feature aimed at reducing eye rotation before it happens.
Bausch & Lomb Zyoptix — compares iris photos to correct for the eye twisting both before and during treatment.

A Closer Look at VISX Iris Registration
The VISX system is worth explaining a bit further, since it's the technology that started all of this. Here's how it works, step by step:
Before surgery, while you're sitting upright, the machine takes a detailed photo of your iris along with your eye measurements.
On the day of surgery, once you're lying down, the machine takes a fresh photo of your iris.
It compares the two photos and works out exactly how your eye has rotated or shifted since your first scan.
It then automatically turns the treatment plan to match your eye's real position — before any laser is applied.
This matters most for people with more noticeable astigmatism, since small errors in direction have a bigger effect on their results. Research comparing this technology against not using it has generally shown it helps keep the treatment lined up properly, which supports more consistent, reliable results.
